What is the quantity and composition of the dossier for 'Settling the enjoyment of occupational accident and disease benefits due to recurrent disabilities'?

Based on point 5.1, clause 5, Section III of the Appendix accompanying Decision No. 896/QD-BHXH of the Director-General of Vietnam Social Security, the quantity of the dossier for 'Settling the enjoyment of occupational accident and disease benefits due to recurrent disabilities' is 01 set; the components of the dossier (if not specified as originals, valid copies can be submitted) include:

1. Original or valid copy of Social Insurance Book (in cases where the contribution period is preserved but there is no data in the system or the insurance code has not been issued) for cases of occupational disability already recognized but not meeting the conditions for a reduction in working capacity to enjoy the benefit.

2. For cases treated before July 01, 2016, but the previous assessment did not meet the conditions for a reduction in working capacity to enjoy occupational accident and disease benefits: Record of occupational accident investigation; for cases where occupational accidents are determined as occupational disabilities, an additional copy of one of the following documents: a copy of the on-site examination report, on-site diagram, or a copy of the traffic accident record by the public security or military criminal investigation agency.

3. For cases receiving occupational accident and disease benefits before January 01, 2007, already equipped with medical devices, orthopedic devices, now requesting additional equipment: Invoices, receipts for the purchase of provided equipment; train or bus tickets for travel to receive the equipment (if any).

4. Record of the last nearest working capacity reduction assessment by the Occupational Diseases and Occupational Accidents Assessment Council for cases that have been assessed but do not meet the conditions for a reduction in working capacity to enjoy the benefit.

5. Reassessment record of the reduction in working capacity after the treatment of relapse or recurrence of occupational accidents and diseases by the Occupational Diseases and Occupational Accidents Assessment Council.

6. Appointment of the healthcare facility, orthopedic facility, or function restoration facility regarding the provision of medical devices, orthopedic devices (if any).

7. Invoices, receipts for the examination fee along with the list of examination contents by the facility performing the assessment (for cases paying the assessment fee).
