Currently, I am using a health insurance card with a subject code of an "Enterprise". Then if I want to convert the object code from "Enterprise" to “TS” (Relatives of people with meritorious services to the revolution who are own parents, spouses or children of matyrs and people who bring up matyrs), what are the needed documents and procedures? Please give me guidance on this.
