I had lost my social insurance book, previously I paid the social insurance premiums from 2005 to 2010. Ever since I don't pay anymore and the company accountant told me that it was lost and the insurance agency did not keep it. I paid insurance premiums when I lived in Phuc Yen and now I moved to Yen Lac. So what can I do to have my social insurance book reissued? Please give me detailed instructions. Sincerely thank you.
Under the provisions of Clause 31, Article 1 of Decision No. 505/QD-BHXH dated March 27, 2020 amending and supplementing a number of articles of the collection process of social insurance, health insurance, unemployment insurance and occupational accident insurance; manage the social insurance books and health insurance cards issued together with the Decision No. 595/QD-BHXH dated April 14th, 2017 of the General Director of Vietnam Social Security, the composition of the dossiers for re-issuance due to loss or damage: Registration declaration, adjustment of information of social insurance and health insurance (Form TK1-TS).
We suggest that you contact the social insurance agency of residence or the social insurance agencies nationwide to be re-issued with the lost social insurance book.
