If the employer requires the issuance of social insurance books for new participants, what does the dossier applying for social insurance participation and social insurance book issuance consist of? If an employee is participating in compulsory social insurance at a company, where does he/she submit his/her dossier and what does the dossier consist of?

 The employer shall receive the dossier compiled by the employee and complete the following forms:

- The declaration of the unit that participates in or revises social or health insurance information (Form TK3-TS).

- The list of employees participating in social insurance, health insurance, unemployment insurance, and labour accident and occupational disease insurance (Form D02-TS).

- The list of relevant information (Form D01-TS).

* Item 1.1 of Clause 1 under Article 31 of Decision No.595/QD-BHXH on the process of collecting social insurance, health insurance, unemployment insurance, and labour accident and occupational disease insurance contributions; and managing social insurance books and health insurance cards, that was issued with Decision No.595/QD-BHXH dated April 14, 2017 by the General Director of the Vietnam Social Security stipulates:

- Participants in compulsory social insurance: Make and submit dossiers to the units where they work.

- Overseas workers under contracts specified in Items a, c, d of Point 1.7 under Clause 1, Article 4 on the process of collecting social insurance, health insurance, unemployment insurance, and labour accident and occupational disease insurance contributions; and managing social insurance books and health insurance cards, that was issued together with Decision No.595/QD-BHXH shall make and submit dossiers as follows: If insurance contributions are paid through the units sending workers abroad, the workers make dossiers and submit them to those units; if contributions are paid directly to the social insurance agencies of the workers’ places of residence before they leave to work abroad, the workers make and submit dossiers to the social insurance agencies; if the workers have their contracts extended or sign new labour contracts in the country they are working in and they pay insurance contributions that should have been paid during that time after returning home, they shall make and submit dossiers to the units they are working in or to the social insurance agencies.

The dossier consists of:

In case of working at the unit: a form registering for participation and adjustment of social insurance or health insurance information (Form TK1-TS).

In case of going to work abroad under a contract specified in Items a, c, d of Point 1.7 under Clause 1, Article 4 of Decision No.595/QD-BHXH: the form registering for participation and adjustment of social or health insurance information (Form TK1-TS); a fixed-term labour contract in a foreign country, or an extended labour contract with the written extension of the labour contract, or a new labour contract signed in the country receiving the worker.
