My company has been reporting the increase of 23 new employees for several times but it is taking forever to be accepted, so may I ask what are the forms needed for new increases? The company is divided into 3 cases as follows: 1. Having a code but no books: What forms do we need for this case? We sent TK1, and D02 but still got rejected. 2. New increases own a book; TK1 and D02 were also sent. 3. New increases (without books) have all declared in TK1 and D02.

Pursuant to the instructions in Article 23 of the collection process of social insurance, health insurance, unemployment insurance, occupational accidents and disease insurance premiums; managing the social insurance books and health insurance cards enclosed with the Consolidated Document No. 2089/VBHN-BHXH dated June 26, 2020 of the Vietnam Social Security; Decision No. 1040/QD-BHXH dated August 18, 2020 of the Vietnam Social Security, promulgating the form of Report on the employment of employees and the list of participants in social insurance, health insurance and unemployment insurance, to report an increase in employees participating in social insurance and health insurance, please provide documents to the social insurance agency including:

1. For employees who have not been issued with social insurance codes:

- Declaration form of participation and adjustment of information on social insurance and health insurance (Form TK1-TS).

- Report on employment situation and list of participants in social insurance, health insurance and unemployment insurance (Form D02-LT).

- In case the employee is entitled to higher health insurance benefits: Supplementary proofs (if any)

2. For employees who have been issued with social insurance codes or already have a social insurance book, they are not required to provide the TK01-TS form and only need the social insurance code given to the employer to fill in the D02-LT form.
