1. Step 1: The insurance participant shall submit a dossier as requested at the “dossier components” and “number of dossier” sections below.
2. Step 2: The social insurance agency
2.1. The single-window section shall receive the dossier from the insurance participant and check the dossier in the terms of components and number; if the dossier is valid and complete, it shall make an appointment for result return and then forward the dossier to the premium collection management division/team; in case the dossier is invalid or incomplete as prescribed, it shall make a note and return the dossier back to the insurance participant.
2.2. The premium collection management division/team shall examine the dossier and coordinate with the planning-finance division/team in determining the refundable premium amount and report such to the director of the social insurance agency.
2.3. The director of the social insurance agency shall issue a premium refund decision (Form C16-TS).
2.4. The planning-finance division/team shall make the refund to the insurance participant.
Submission in person or sending by post to the single-window section
a/ A declaration providing or adjusting information on social or health insurance participants (Form TK1-TS).
b/ The social insurance book, for persons participating in social insurance on a voluntary basis; valid health insurance card, for persons participating in health insurance (except cases in which the insurance participants have died);
c/ A certified copy of the death certificate or a copy of the death certificate enclosed with the original, in case the insurance participant has died);
d The premium refund decision (Form C16-TS).
Within 15 days after receiving a complete dossier under regulations
- Declaration providing or adjusting information on social or health insurance participants (Form TK1-TS).
- Premium refund decision (Form C16-TS).
