- Step 1: The applicant submits a dossier directly to the provincial-level social insurance agency or district-level social insurance agency or sends a dossier by post to the provincial-level social insurance agency

- Step 2: The district-level social insurance agency receives the dossier from the applicant and transfers it to the provincial-level social insurance agency

+ The provincial-level social insurance agency receives the dossier from the applicant or from the district-level social insurance agency; processes the dossier and returns the processed dossier to the district-level social insurance agency

+ The district-level social insurance agency receives the processed dossier from the provincial-level social insurance agency for returning it to the applicant

Submitting the dossier directly at, or by post to, the single-window division of the district-level social insurance agency or the division for receiving and notifying administrative procedure settlement results of the provincial-level social insurance agency

Profile component

1. For an employee requesting payment of monthly allowance under Decision No. 91/2000/QD-TTg:

a/ A written request for payment of monthly allowance (the original), made by the applicant and certified by the administration of the locality where he/she resides (for a person who used to receive working-capacity loss allowance prior to July 1, 2000), or a written request for payment of funeral allowance (the original), made by the applicant’s relative and certified by the administration of the locality where the applicant resides;

b/ A dossier for payment of working-capacity loss allowance must comprise:

- A decision (the original) issued by the agency or unit that permits the employee to cease working due to working-capacity loss;

- A slip (the original) on the employee’s personal information, in case information on his/her working process is not shown in the back page of the above decision;

- A medical assessment record (the original), (unless the employee ceases working due to working-capacity loss under Resolution No. 16-HDBT of February 8, 1982);

In case of availability of the original excerpts of the dossier on the employee’s working-capacity loss granted by the Ministry of Labor, War Invalids and Social Affairs is available, they may be used instead of the above dossier on working-capacity loss.

c/ A decision on discontinuation to enjoy working-capacity loss allowance; in the absence of this decision, the employee must state in the written request that he/she will not resume enjoying any social insurance allowance after discontinuing to enjoy monthly working-capacity loss allowance;

d/ A death certificate or death notice, in case of requesting payment of funeral allowance

2. For an employee requesting payment of monthly allowance under Decision No. 613/QD-TTg:

a/ A written request for payment of monthly allowance (Form No. 1- QD 613, the original);

b/ If the dossier on enjoyment of working-capacity loss allowance managed by the social insurance agency is unavailable or incomplete, the employee shall, on case-by-case basis, additionally submit the following papers:

- A decision (the original) issued by the agency or unit that permits the employee to cease working due to working-capacity loss;

- A slip (the original) on the employee’s personal information, in case information on his/her working process is not stated in the back page of the above decision;

- A medical assessment record ((the original), (unless the employee ceases working due to working-capacity loss under Resolution No. 16-HDBT of February 8, 1982;

- A decision or notice on discontinuation to enjoy monthly working-capacity loss allowance.

In case of availability of the original excerpts of the dossier on the employee’s working-capacity loss granted by the Ministry of Labor, War Invalids and Social Affairs is available, they may be used instead of the above dossier on working-capacity loss.

c/ For a person who is sentenced to imprisonment but not entitled to suspended sentence; a person who was declared missing by the court and reappears; or an illegal emigrant who repatriates to lawfully settle in the country, the dossier must additionally comprise:

- A letter of authorization (Form No. 13-HSB, the original), for a person who has been serving his/her imprisonment penalty since January 1, 2016;

- A certificate of complete serving of the imprisonment penalty or a written amnesty on early release or a decision on exoneration or reprieve from judgment execution for a person who started to served his/her imprisonment penalty between January 1, 1995, to December 31, 2015;

- A competent state agency’s document permitting the illegal emigrant to repatriate to lawfully settle in the country;

- The court’s legally effective decision annulling the decision on missing declaration, for a person declared missing by the court who reappears.

Note: If the originals of the above dossier components are not required, the applicant may submit their originals, copies enclosed with the originals for comparison, certified copies or duplicates.
