1. Step 1: Employees shall make the prescribed dossiers and send them directly to social insurance agencies of districts where they are on pension 2. Step 2: District-level social insurance agencies shall check and compare the dossiers and if fully satisfying conditions, the dossiers will later be received and transferred to provincial-level social insurance agencies 3. Step 3: Provincial-level social insurance agencies shall check and compare the dossier which will later be processed if fully satisfying the conditions
Sending directly to single-window divisions of district-level social insurance agencies
1. Adjusted information on personal identity of persons who are on pension and monthly social insurance allowance: - The application, made according to Form No. 14-HSB (the original); - Birth certificate or birth certificate extract or civil status change, correction or supplementation extract, ethnicity re-determination. 2. Adjusted or annulled decisions on enjoyment of social insurance or decisions on termination of enjoyment of social insurance - The application, made according to Form No. 14-HSB (the original), in case an employee wants to make adjustment; - The dossier, papers and documents serving the adjustment, annulment of a decision on enjoyment or termination of enjoyment of social insurance Note: In case the originals of the mentioned dossier components are not required, the originals, copies enclosed with the originals for comparison, certified copies or copies issued from the original book may be submitted.
