Ghi chú:  - (1) Đối với trường hợp hưởng từ ngày 01/7/2016 trở đi thì thay bằng Luật An toàn, vệ sinh lao động số 84/2015/QH13.

                 - (2) Ghi niên hạn theo quy định; trường hợp lắp mắt giả thì không thể hiện niên hạn;

                - (3) Ghi theo tên đơn vị hành chính cấp huyện.

Note: - (1) For persons enjoying the allowance from July 1, 2016, Law No. 84/2015/QH13 on Occupational Safety and Health shall be appllied.

          -(2) To write useful life in accordance with law; not to write useful life in case of fitting prosthetic eye;

           - (3) To write the name of the district-level adminsitrative unit.

            Trường hợp có sự khác nhau về cách hiểu giữa tiếng Việt và tiếng Anh trong văn bản, tiếng Việt là ngôn ngữ chính và là căn cứ pháp lý để giải quyết tranh chấp./ If there are differences understanding between Vietnamese and English in this paper, VietNamese will be considered the primary language and legal basis to resolve the dispute.
