1. Step 1: Medical examination and treatment establishment: - Prepares a dossier with the components and number as required below and submits it to the social insurance agency 2. Step 2: Social insurance agency: - Receives the dossier, appraises and determines conditions for organization of the health insurance-covered medical examination and treatment by the medical examination and treatment establishment. - If the medical examination and treatment establishment fully satisfies the conditions for contract signing, organizes the signing of a contract on health insurance-covered medical examination and treatment (according to the form of contract on health insurance-covered medical examination and treatment provided in Appendix No. 03 to Joint Circular No. 41/2014/TTLT/BYT-BTC of November 24, 2014, of the Ministry of Health and the Ministry of Finance); or issues a written reply clearly stating the reason, if the medical examination and treatment establishment fails to fully satisfy the conditions for contract signing.
- Submitting the dossier directly at the social insurance agency or by post; - Signing the contract at the office of the social insurance agency.
1. For a medical examination and treatment establishment which signs for the first time the contract on health insurance-covered medical examination and treatment or which has halted the contract on health insurance-covered medical examination and treatment for 6 months or longer: - The medical examination and treatment establishment’s written request for signing a contract on health insurance-covered medical examination and treatment; - Copy of the medical examination and treatment establishment’s operation license; - Copy of a competent authority’s decision on hospital classification (if any); - A competent authority’s decision on expertise and technical level, for a non-public medical examination and treatment establishment. 2. For a medical examination and treatment establishment which annually signs a contract on health insurance-covered medical examination and treatment: To supplement its functions, tasks, scope of expertise and hospital grade approved by a competent authority (if any).

The medical examination and treatment establishment satisfies the conditions prescribed in Article 42 of the Law on Medical Examination and Treatment, specifically as follows: - To have an establishment decision issued by a competent state agency, for a medical examination and treatment establishment set up by the State, or a business registration certificate or investment license as prescribed by law, for another medical examination and treatment establishment. - To possess an operation license granted by the Minister of Health, the Minister of National Defense or the director of a provincial-level Health Department.
- Document consolidating Health Insurance Law No. 01/VBHN-VPQH of July 10, 2014, effective on January 1, 2015. - The Government’s Decree No. 105/2014/ND-CP of November 15, 2014, detailing and guiding the implementation of a number of articles of the Law On Health Insurance. - Joint Circular No. 41/2014/TTLT/BYT-BTC of November 24, 2014, of the Ministry of Health and the Ministry of Finance, guiding the implementation of health insurance. - Decision No. 1399/QD-BHXH of December 22, 2014, of the General Director of Vietnam Social Security, on organization of implementation of health insurance in medical examination and treatment. - Vietnam Social Security’s Decision No. 1018/QD-BHXH of October 10, 2014, amending a number of contents of decisions promulgating regulations on management of social insurance and health insurance revenues and expenditures.
