Is 3rd party could act refund application on my behalf ? 4. could you send me application form (or provide linkage for me to download) and/or authorization form. Or the forms are available only at your office ? (should I sign personally on the authorization form or authorization could be done via internet ? ) Please kindly confirm your office address shown below is correct?
