Step by step

1. Step 1: The insurance buyer:

- Submits a dossier to the social insurance agency or through his/her employer, for working employees.

- Submits a dossier to an authorized premium collector or the district social insurance agency, for persons buying voluntary social insurance and health insurance.

- Submits a dossier to the social insurance agency, for persons currently reserving the social insurance premium payment period or having been settled for enjoyment of pension or allowance.

2.  Step 2: The employer, authorized premium collector or agency managing the concerned insurance buyer makes a dossier as prescribed and submits it to the social insurance agency.

3. Step 3: The social insurance agency:

- Receives the dossier, checks its components and number, if the dossier is valid, writes an appointment slip for result notification and collects charge (for re-grant or exchange of a lost, torn or damaged card). If the dossier is invalid, fully writes the reason and returns the dossier to the insurance buyer or his/her employer or the authorized premium collector.

- Returns the dossier; social insurance book or health insurance card to the insurance buyer or his/her employer or the authorized premium collector on the deadline written in the appointment slip for result notification.

Process

Submitting dossiers and receiving results of processing of administrative formalities at the social insurance agency, through e-transactions or by post.

1. Direct submission to the social insurance agency

1.1 Re-grant, or incorporation or adjustment of information of, a social insurance book

a) Re-grant of a social insurance book due to loss, including re-grant of the entire social insurance book; re-grant of the social insurance book’s cover or re-grant of the social insurance book’s separate sheets

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Social insurance book (in case the insurance buyer only loses 1 sheet or some separate sheets of the social insurance book).

b) Re-grant of a damaged social insurance book, including re-grant of the entire social insurance book; re-grant of the social insurance book’s cover; re-grant of the social insurance book’s separate sheets

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Cover of the social insurance book (in case the social insurance book’s cover is damaged).

- Social insurance book (in case the social insurance book is damaged or 1 sheet or some separate sheets of the social insurance book are damaged).

c) Re-grant of a social insurance book due to incorporation of social insurance books or change of serial number of social insurance book

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Social insurance book of which the serial number is requested for change (for change of the social insurance book’s serial number)

- Granted social insurance books (for incorporation of social insurance books)

d) Re-grant of a social insurance book due to change of family name, last name or middle name; birth date; gender, ethnicity; citizenship

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Social insurance book.

- Birth certificate or a competent agency’s decision on adjustment or correction (certified copy or copy enclosed with the original for comparison).

dd) Adjustment of contents written on a social insurance book (except for adjustment of performance of hard, hazardous or dangerous jobs or particularly hard, hazardous or dangerous jobs before January 1, 1995)

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS);

- Social insurance book;                                                         

- Dossier comprising one of the following papers (certified copy or copy enclosed with the original for comparison): Decision on job assignment; Decision on salary enjoyment; Decision (document) on working place assignment or transfer; Labor contract, Work contract.

1.2. Re-grant or exchange of a health insurance card

a) Re-grant of a health insurance card due to loss or damage

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card (for re-grant of a damaged health insurance card).

b) Re-grant of a health insurance card due to change in registered primary medical examination and treatment establishment

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card.

c) Re-grant of a health insurance card due to supplementation or adjustment of the code of place of residence of the card holder

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS);

- Health insurance card;

- Residence registration book or temporary residence book (certified copy or copy enclosed with the original for comparison).

d) Exchange of a health insurance card due to change of family name, last name or middle name; birth date; gender

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS);

- Health insurance card;

- Birth certificate or a competent agency’s decision on adjustment or correction (certified copy or copy enclosed with the original for comparison).

dd) Exchange of a health insurance card according to the enjoyment level applicable to persons with meritorious services to the revolutionary under the Ordinance on preferential treatment for persons with meritorious services to the revolution

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card.

- Accompanied supporting dossier (certified copy or copy enclosed with the original for comparison):

dd1) For a person engaged in revolutionary activities before January 1, 1945; person engaged in revolutionary activities between January 1, 1945 and before the general uprisings of August 19, 1945; Vietnamese heroic mother; war invalid, person enjoying policies like war invalids, grade-B war invalid; or diseased army man suffering a working capacity deficiency rate of 81% or higher, a dossier must comprise one of the following papers:

- Decision recognizing a person engaged in revolutionary activities before January 1, 1945; or a person engaged in revolutionary activities between January 1, 1945 and before the general uprisings of August 19, 1945, issued by the Party standing committee of a province or centrally run city;

- War invalid or diseased army man card;

- Decision granting a certificate of war invalid or person enjoying policies like war invalids and monthly allowance or benefit

- Decision on monthly allowance or benefit issued by the provincial-level Labor, War Invalids and Social Affairs Department;

- Written certification by the provincial-level Labor, War Invalids and Social Affairs Department of the locality where the concerned person is enjoying monthly allowance or has received lump-sum allowance (under guidance on grounds for identifying persons with meritorious services to the revolution specified in Official Letter No. 467/NCC of June 17, 2010 of the Ministry of Labor, War Invalids and Social Affairs).

dd2) For a person with meritorious services to the revolution under the Ordinance on preferential treatment for persons with meritorious services to the revolution, other than those specified at point dd1 above, a dossier must comprise one of the following papers:

- Document proving the concerned person has been awarded the Resistance War Order or Medal; or the Victory Order or Medal;

- War invalid or diseased army man card;

- Decision granting a certificate of war invalid or person enjoying policies like war invalids and monthly allowance or benefit;

- Certificate of commendation of resistance war achievements and duration of participation in resistance war activities issued by the district-level emulation and commendation agency;

- Decision on monthly allowance or benefit issued by the provincial-level Labor, War Invalids and Social Affairs Department;

- Written certification by the provincial-level Labor, War Invalids and Social Affairs Department of the locality where the concerned person is enjoying monthly allowance or has received lump-sum allowance (under guidance on grounds for identifying persons with meritorious services to the revolution specified in Official Letter No. 467/NCC of June 17, 2010 of the Ministry of Labor, War Invalids and Social Affairs).

e) Exchange of a health insurance card according to the enjoyment level applicable to war veterans under the Ordinance of War Veterans

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card.

- Accompanied supporting dossier (certified copy or copy enclosed with the original for comparison):

e1) For officers and men of Vietnam People’s Army (comprising officers, career army men, non-commissioned officers, soldiers) being personnel of the regular army, the local army, the border-guard force or the special task force, having participated in resistance wars against foreign aggression for national defense or performed international duties; officers and men of the militia, self-defense or guerilla force in the South and defense workers engaged in fighting against foreign aggression from April 30, 1975 or earlier, members of armed groups working in regions temporarily occupied by the enemy from July 20, 1954 or earlier; officers and men of the militia, self-defense or guerilla force in the North from January 27, 1973 or earlier; and persons having enjoyed allowance under Decision No. 290/2005/QD-TTg of November 8, 2005; Decision No. 188/2007/QD-TTg of December 6, 2007; Decision No. 142/2008/QD-TTg of October 27, 2008; Decision No. 38/2010/QD-TTg of May 6, 2010, a dossier must comprise one of the following papers:

- Decision on discharge from the army or demobilization or transfer to another sector;

- Decision on enjoyment of allowance under Decision No. 290/2005/QD-TTg of the Prime Minister;

- Decision on enjoyment of allowance under Decision No. 188/2007/QD-TTg of the Prime Minister;

- Decision on enjoyment of allowance under Decision No. 142/2008/QD-TTg of the Prime Minister;

- Decision on enjoyment of allowance under Decision No. 38/2010/QD-TTg of the Prime Minister.

e2) For army men, defense workers and employees, men of the  militia or self defense force directly engaged in fighting in war to protect the fatherland on the southwestern border, northern border or on international mission in Cambodia or assistance to Laos after April 30, 1975, under the Prime Minister’s Decision No. 62/2011/QD-TTg of November 9, 2011, a dossier must comprise one of the following papers: decision on discharge from the army, demobilization or transfer to another sector, which clearly states the time and place of stationing under Decision No. 62/2011/QD-TTg, or decision on enjoyment of allowance under Decision No. 62/2011/QD-TTg.

e3) For officers, career army men having fulfilled duties while in active service who have retired, been discharged from the army or transferred to other sectors: Decision on discharge from the army, demobilization or transfer to another sector.

g) Exchange of a health insurance card according to the enjoyment level applicable persons directly engaged in the war of resistance against America; persons engaged in the war to protect the Fatherland, on international mission in Cambodia and assistance to Laos after April 30, 1975; vanguard youth in the war of resistance against France, and under the Prime Minister’s Decision No. 40/2011/QD-TTg of July 27, 2011, on regimes for vanguard youth having fulfilled duties in the resistance war as prescribed under the Prime Minister’s decisions (other than persons specified at Points dd and e above)

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card.

- Accompanied supporting dossier, comprising one of the following papers (certified copy or copy enclosed with the original for comparison):

+ Certificate of vanguard youth issued by a competent agency before the vanguard youth came back to the locality under the Labor, War Invalids and Social Affairs Ministry’s Circular No. 24/2009/TT-BLDTBXH of July 10, 2009, and Circular No. 08/2012/TT-BLDTBXH of April 16, 2012;

+ Certificate of fulfillment of duties of vanguard youth issued by a competent agency before the vanguard youth came back to the locality under Circular No. 24/2009/TT-BLDTBXH, and Circular No. 08/2012/TT-BLDTBXH;

+ Certificate of merit during the time of participation in the vanguard youth force issued by a competent agency before the vanguard youth came back to the locality under Circular No. 24/2009/TT-BLDTBXH, and Circular No. 08/2012/TT-BLDTBXH;

+ Resume of official or party member, stating the concerned person is vanguard youth under Circular No. 24/2009/TT-BLDTBXH;

+ Decision on enjoyment of allowance under Decision No. 290/2005/QD-TTg of the Prime Minister;

+ Decision on enjoyment of allowance under Decision No. 188/2007/QD-TTg of the Prime Minister;

+ Decision on enjoyment of allowance under Decision No. 142/2008/QD-TTg of the Prime Minister;

+ Decision on enjoyment of allowance under Decision No. 38/2010/QD-TTg of the Prime Minister;

+ Decision on enjoyment of allowance under Decision No. 62/2011/QD-TTg of the Prime Minister;

+ Decision on enjoyment of allowance under Decision No. 40/2011/QD-TTg of July 27, 2011 of the Prime Minister;

+ Decision on enjoyment of allowance under Decision No. 53/2010/QD-TTg of August 20, 2010 of the Prime Minister.

h) Exchange of a health insurance card according to the enjoyment level applicable to natural father, natural mother, wife or husband and children of a war martyr or a person raising up a war martyr

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card.

- Accompanied supporting dossier, comprising one of the following papers (certified copy or copy enclosed with the original for comparison):

+ The provincial-level Labor, War Invalids and Social Affairs Department’s certificate of family of war martyr under the Labor, War Invalids and Social Affairs Ministry’s Circular No. 05/2013/TT-BLDTBXH of May 15, 2013.

+ Certificate of the provincial-level Labor, War Invalids and Social Affairs Department which receives and manages the dossier and carries out procedures for buying health insurance for the war martyr’s relative or person raising up the war martyr under Circular No. 05/2003/TT-BLDTBXH.

i) Exchange of a health insurance card according to the enjoyment level applicable to ethnic minority inhabitants of areas with difficult socio-economic conditions; inhabitants of areas with extremely difficult socio-economic conditions; inhabitants of island communes and districts, a dossier must comprise one of the following papers:

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card.

- Accompanied supporting dossier, comprising one of the following papers (certified copy or copy enclosed with the original for comparison):

+ Household registration book;

+ Temporary residence book;

+ Certificate of the commune-level People’s Committee of the locality of residence.

k) Exchange of a health insurance card according to the enjoyment level applicable to relatives of persons with meritorious services to the revolution (except for natural father, natural mother, wife or husband, and children of a war martyr; or person raising up a war martyr)

- Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS).

- Health insurance card.

- Accompanied supporting dossier (certified copy or copy enclosed with the original for comparison):

- Certificate of the provincial-level Labor, War Invalids and Social Affairs Department which receives and manages the dossier and carries out procedures for buying health insurance for relatives of a person with meritorious services to the revolution under the Labor, War Invalids and Social Affairs Ministry’s Circular No. 05/2013/TT-BLDTBXH.

2. Dossier submission through employers or authorized premium collectors

- An insurance buyer submits a dossier prescribed at Point 1.1 to his/her employer or authorized premium collector.

- The employer or authorized premium collector makes and submits a dossier to the social insurance agency, which comprises:

+ Declaration providing and changing information of the social insurance or health insurance buyer (form TK1-TS) (in case no supporting dossier is accompanied).

+ Dossier list (in case supporting dossiers are accompanied).

Term of settlement

- For re-grant of a social insurance book due to change of the book’s serial number, birth date, gender, citizenship; or loss, damage or incorporation of the book: within 15 working days (or within 45 working days, in case verification of social insurance premium payment period is complex).

- For modification of contents of a social insurance book: within 10 working days.

- Fore re-grant or exchange of a health insurance card: within 5 working days after receiving a complete dossier as prescribed.
